The Total Aging In Place Program helps participants remain as independent as possible while living safely in the comfort of their own home. The program is a managed long term care plan and provider of long term care services. The program is designed to help participants to continue to live safely in the community in their own homes or apartments by providing participants with an array of health care services that they can receive at our Day Center, in their home, or in a nursing home should that become medically necessary. Services you may receive at home may include home care and home delivered meals. We will also provide you with transportation to and from your home and our day center and to and from medical appointments. Apartments at Stovroff Towers on Weinberg Campus are available to enrollees in the program.
